I created a fourth planet in my second system my first time playing the game. Something went wrong and the game created two copies of the same planet... I am pretty sure at the same location. I loaded up the build queue as soon as I landed my colony ship, then exited, then it started the opening screen again, and so I told it what I wanted to build, thinking something had gone wrong and I had to do it over, nope, two complete planets, both on my planet screen, took a screen shot and saved it if you want to see it in game.
